In order for us to represent you in a personal injury case, we really need your help with the following.  First, we need you to fill out the following forms.  We’ve placed an “X” on each form where we need you to sign/complete the information.  Don’t worry about anything else on the forms, because we will take care of that.  Please return all signed forms to our office.

We also need to make sure you review the “Personal Injury Case Checklist” to make sure you’ve given us ALL the information requested on the checklist.

1) Retainer Agreement – This form is basically the contract between the attorney and the client.  You will notice the attorney receives no fee unless you recover compensation.  Please review carefully and sign where indicated.  If there is more than one person we are representing in this case, each person should fill out their own retainer form.

2) HIPAA Authorization – This form allows us to access your medical records from your health care providers.  Again, each person should fill out his/her form.

3) Agency Authorization – This allows our office to talk to all other parties including insurance companies etc.  It basically says you are hiring our office.  Each person should sign his/her own form.

4) Police Report/Employment Authorization – In some cases, we need to order a police report and/or obtain employment information.

5) Personal Injury Checklist – It is very important that you review this form and make sure we have ALL the information requested on this form.  We cannot effectively represent you unless we have all applicable information.  In some cases, the information requested may not apply to your matter.
